Pelvic ultrasound may be useful when evaluating symptoms such as pelvic pain, abnormal or heavy uterine bleeding, painful periods, suspected fibroids or ovarian cysts, and other gynecologic concerns.

LuxER provides transabdominal pelvic ultrasound only. We do not perform transvaginal ultrasound.

What Can a Pelvic Ultrasound Evaluate?

A transabdominal pelvic ultrasound can provide information about structures within the female pelvis, including:

Uterus

The examination may assess:

  • Uterine size and shape

  • Uterine position

  • Fibroids when visible

  • Other uterine masses or abnormalities that can be identified transabdominally

Endometrium

The uterine lining may be evaluated for:

  • Endometrial thickness and appearance when adequately visualized

  • Findings that may warrant additional gynecologic evaluation

Some abnormalities of the uterine cavity, including small polyps or subtle endometrial findings, may require more detailed imaging that is not available with a transabdominal-only examination.

Ovaries & Adnexal Regions

When adequately visualized, the examination may assess:

  • Ovarian size and appearance

  • Ovarian cysts

  • Adnexal cysts or masses

  • Other visible ovarian or adnexal abnormalities

Visualization of the ovaries can vary depending on anatomy, bowel gas, bladder filling, body habitus, and other individual factors.

Pelvic Free Fluid

Ultrasound may identify free fluid within the pelvis when present and visible.

IUD Position

An intrauterine device may sometimes be visualized transabdominally.

If the IUD cannot be adequately seen or its exact position cannot be confidently determined, your clinician may recommend additional gynecologic imaging, which may include transvaginal ultrasound at another facility.

Who May Benefit From a Pelvic Ultrasound?

A pelvic ultrasound may be appropriate for patients experiencing or evaluating:

Can a Pelvic Ultrasound Detect Fibroids?

Yes, many uterine fibroids can be identified with pelvic ultrasound.

The examination may demonstrate the size and location of visible fibroids and changes in uterine contour.

However, the ability to detect very small fibroids or abnormalities within the uterine cavity may be more limited with transabdominal imaging alone.

If additional detail is necessary, your OB-GYN may recommend specialized imaging.

Can a Pelvic Ultrasound Detect Ovarian Cysts?

Yes.

When the ovaries are adequately visualized, pelvic ultrasound can identify many ovarian cysts and other ovarian or adnexal masses.

If an abnormality is found, the radiologist may recommend follow-up ultrasound, MRI, gynecologic consultation, or other evaluation depending on the finding.

Can This Ultrasound Diagnose PCOS?

A pelvic ultrasound may show ovarian features that can occur with polycystic ovary syndrome (PCOS), but ultrasound alone does not diagnose PCOS.

PCOS is diagnosed using a combination of clinical history, menstrual patterns, laboratory findings, and other criteria evaluated by your healthcare provider.

Can This Ultrasound Rule Out Ovarian Torsion?

No ultrasound examination should be relied upon by itself to completely rule out ovarian torsion.

If ovarian torsion is clinically suspected—particularly with sudden severe one-sided pelvic pain, nausea, or vomiting—urgent emergency evaluation is appropriate.

Do not delay emergency care to schedule an outpatient pelvic ultrasound.

Is This a Transvaginal Ultrasound?

No.

LuxER performs transabdominal pelvic ultrasound only.

A clear ultrasound gel is applied to the lower abdomen, and the sonographer moves the ultrasound transducer externally over the skin to obtain images of the pelvic organs.

There is no internal or vaginal portion of the examination.

Because transvaginal ultrasound can provide more detailed visualization of certain pelvic structures, your radiology report or healthcare provider may recommend additional imaging elsewhere if the transabdominal examination does not provide sufficient information.

When clinically appropriate, a limited pelvic Doppler evaluation may be available for an additional $75 to assess blood flow in selected structures.

A limited Doppler examination does not replace emergency evaluation when conditions such as ovarian torsion are suspected.

How to Prepare for a Transabdominal Pelvic Ultrasound

A comfortably full bladder is important for a transabdominal pelvic ultrasound because it helps create an acoustic window that can improve visualization of pelvic structures.

Before Your Appointment

  • Drink approximately 24–32 ounces of water 45–60 minutes before your ultrasound

  • Do not urinate before the examination unless instructed otherwise

  • Wear comfortable clothing, preferably a two-piece outfit

  • Bring any relevant previous pelvic ultrasound, CT, MRI, or gynecologic reports if available

  • Bring information about your menstrual cycle, including the first day of your last menstrual period when relevant

What Are the Limitations of a Transabdominal Pelvic Ultrasound?

Transabdominal ultrasound provides a useful overview of the pelvis, but some pelvic structures and abnormalities may not be visualized as clearly as they would be with transvaginal or other specialized imaging.

Image quality may be affected by:

  • Bowel gas

  • Bladder filling

  • Body habitus

  • Position of the uterus or ovaries

  • Size or location of an abnormality

  • Other individual anatomical factors

If an organ or area cannot be adequately evaluated, the radiologist may recommend additional imaging or evaluation by your OB-GYN.

A limited examination does not necessarily mean that an abnormality is present—it means additional imaging may be needed to answer the clinical question.

Is Pelvic Ultrasound Safe?

Yes.

Diagnostic ultrasound uses sound waves rather than ionizing radiation to create images of internal structures.

The examination is non-invasive and generally well tolerated.

When to Seek Immediate Medical Care

An outpatient pelvic ultrasound should not delay emergency evaluation for serious or rapidly worsening symptoms.

Seek urgent or emergency medical care for symptoms such as:

  • Sudden or severe pelvic or abdominal pain

  • Severe one-sided pelvic pain, particularly with nausea or vomiting

  • Heavy vaginal bleeding accompanied by dizziness, weakness, or fainting

  • Fever with significant pelvic pain

  • Fainting or near-fainting

  • Severe or rapidly worsening symptoms

  • Pregnancy-associated pelvic pain or bleeding that may require urgent evaluation

Do I need a full bladder?

An IUD may sometimes be visualized transabdominally. If its position cannot be confidently assessed, additional gynecologic imaging may be recommended.

Can this scan diagnose PCOS?

Not by itself. Ultrasound can demonstrate ovarian features associated with PCOS, but the diagnosis requires consideration of other clinical and laboratory findings.

Can this scan rule out ovarian torsion?

No. Ultrasound findings alone cannot completely exclude ovarian torsion. Sudden severe pelvic pain—especially with nausea or vomiting—requires urgent medical evaluation.

Does the ultrasound use radiation?

No. Ultrasound uses sound waves and does not expose you to ionizing radiation.

Do I need a physician referral?
